Who We Serve

Who Should Apply to Live at Thrive?

Thrive Living is a supported living community for adults with and without disabilities who are looking for a warm, supportive community in which to live. Thrive Living is unique because it provides maximum independence for our residents with disabilities via the support of onsite support staff and an executive director who coordinates the details that keep Thrive safe, nurturing, and supportive.  Onsite staff will be floating throughout the community during morning and evening hours to ensure that the residents effectively get to their jobs or day programs, and then have support during the evenings to ensure a relaxing wind down to their day. It is our goal to have an overnight, on-call staff person onsite for emergencies.

Thrive Living Resident Requirements:

  • Adult, aged 25-45 at time of move in, and a high school graduate
  • Expresses a desire to live on their own
  • Understands and can abide by Thrive’s code of conduct policy
  • Is not violent to self or others
  • Accepts redirection and suggestions without significant difficulty
  • Able to perform dressing, grooming, eating, mobility with a maximum of minimal assistance (the resident completes >75% of the task). Requires less than 30 minutes of 1:1 time daily, and an additional 60 minutes once/week for additional tasks of cleaning, laundry, taking a shopping inventory, making appointments, refilling medication minders, meal planning/prepping, etc.
  • Participates in a day program, work program, employment or volunteering >/= 20 hours per week
  • Has a desire to live in a community where socialization and continuous personal development are encouraged
  • Is willing to use technology whenever it is safe and appropriate to build independence
  • Expects to be treated with respect and treats others with respect
  • Has an alternative residence for sudden and/or unexpected medical situations that require care beyond the scope of which Thrive provides
  • Understands that we are a community and watches out for others, and understands that they can and should bring their concerns to our staff, then Executive Director or Resident Liaison
  • Successfully completes the interview and trial stay, and has the approval of the executive director

Respite Care Client Requirements:

  • Adult, aged 22-35 at time of move in
  • Expresses a desire to spend time at Thrive Respite Care
  • Understands and can abide by Thrive’s code of conduct policy
  • Is not violent to self or others
  • Accepts redirection and suggestions without significant difficulty
  • Able to perform dressing, grooming, eating, mobility with a maximum of 1 person (moderate) assistance, meaning that the client performs at least 50% of the tasks or has specific equipment that performs at least 50% of the effort to complete the task. If equipment is required i.e. for safe transfers, the client must be able to operate any assistive devices they use or be able to instruct our staff in how to safely use this equipment.
  • Is able to privately pay for their services while in our care
  • Thrive Respite Care does not provide emergency respite care.  We provide scheduled respite care only, and have the right to refuse care to people who don’t meet our eligibility requirements or if our facility does not have sufficient staffing to provide what we determine to be an acceptable level of staffing.
  • Thrive Respite Care reserves the right to cancel a respite stay in cases of staffing shortages, illness, and other unforeseen circumstances.

Who We Don’t Serve

Thrive IS NOT an appropriate living place for people with primary psychiatric diagnoses, mental illness, behavioral issues, wandering, significant medical care needs, and those people who require 1:1 care to ensure the safety of the individual or others in their proximity. Thrive will not necessarily have a nurse on staff. Thrive is not an ideal setting for those who don’t respond to redirection in a reasonable manner, or those who cannot understand and adhere to the code of conduct of our community. As with anyone who is moving out on their own, there are inherent risks, however, Thrive’s structure, resident screening process, and individualized planning should minimize this risk.
